-
Claim Your Credit
Claim your qualifying contribution on your Arizona state income tax return using Forms 321 and 301. Use MentorKids USA QCO Code: 20325.

BEYOND THE TAX CREDIT
YOUR GIFT HELPS BUILD WHAT LASTS.
At MentorKids, leadership grows over time.
In our Promise Neighborhoods, students receive academic support, grow in faith and character, and build relationships with caring adults who keep showing up.
As they get older, they're invited to lead, too. Through iLEAD, high school students mentor younger children—passing along the same encouragement and support they've received.
Your Arizona tax credit gift helps keep this year-round pathway free for students and families.
